They typically cost about $8,000 to $30,000. Most rely on bulky gas chromatography and mass spectroscopy systems to identify airborne compounds. They generally require several breaths saved in a balloon. Other breath-analyzing devices, such as alcohol Breathalyzers, use technologies that are not sufficiently accurate for medical use.
An e-nose's ability to recognize a particular odor and flag it as a warning sign has been hindered by lack of specificity?that is, the inability to detect and quantify a single substance in an exhalation that contains hundreds of compounds. Several new approaches aim to overcome that problem. One promising project involves a team of? Stony Brook University, The State University of New York, researchers led by associate professor of materials science and engineering Perena Gouma, who have developed an e-nose that uses semiconducting ceramic sensors to detect disease and measure metabolic functions.
According to Gouma, the sensor is highly sensitive to gaseous disease-signalling markers in the breath?it can detect specific molecules in parts per billion. The sensor chip is covered with spaghettilike metal oxide fibers that have diameters ranging from one to 100 nanometers (a nanometer is one billionth of a meter). The nanofibers' crystalline structure and the atomic configuration on their surfaces determine which compound can be detected. For instance, gases such as carbon monoxide and methane stick to a nanofiber oxide sensor whose atoms have a tetragonal (four-sided) arrangement. Nanofibers can be made in large quantities and tweaked during production to respond to different compounds. Once the e-nose detects its target molecule, its concentration is shown on a digital display.
Breath contains hundreds of volatile organic compounds that are by-products of our metabolism. Certain diseases alter the mix of gases, introducing new compounds or altering the levels of existing ones, Gouma says. One sensor could detect acetone in the breath of diabetics to determine whether they need to increase their insulin doses. Another could measure nitric oxide in asthmatics' breath to detect an impending or worsening asthma attack.
Gouma's group is also developing e-noses to detect the organic compound isoprene to measure cholesterol levels, ammonia to detect kidney disease, and a mix of ammonia and ketones to get a whiff of liver disease. The researchers are also developing an e-nose that can distinguish between two similar chemical structures?alkanes and alkenes?for early detection of lung cancer, Gouma says.
These prototype e-noses resemble hardcover novels with a mouthpiece attached at one end. Gouma anticipates the final design will be about the size of a key ring. She expects at least one of her e-noses will be ready for clinical testing within a year. Unlike other e-noses, Gouma's require only a single breath to detect specific compounds, she says. This means for some conditions breath tests can be performed in lieu of blood samples. Gouma anticipates such handheld e-noses such as these could cost as little as $20 each.
Nathan Lewis, a chemistry professor at the California Institute of Technology, is also developing e-nose technology. His system relies on polymer films rather than nanoscale wires to identify compounds. But he says with respect to Gouma's gizmos: "Ceramic nanosensing technology is one promising approach for medical and other electronic-nose applications. [Gouma's] work is a significant step forward in that regard."
Other researchers, however, are skeptical about an e-nose's ability to analyze individual components of even simple mixtures of volatile organic compounds (VOC). "Among the challenges to obtaining clinically useful measurements of breath VOCs, such as acetone or other possible disease markers, one of the toughest is reliably differentiating the marker or markers from the background VOCs found in breath," says Edward Zellers, an environmental health sciences professor at the University of Michigan. He adds, "This has not yet been demonstrated with any e-nose technology of which I am aware." Still, although he is wary of e-noses being used for disease diagnosis, Zellers says Gouma's technology has promise as a means of detecting individual reactive inorganic markers such as nitric oxide and ammonia.
As Hippocrates may have attested, promising medical ideas frequently do not come to pass. Patients should not hold their breath in anticipation of inexpensive handheld e-noses until the technology proves it is up to snuff in clinical trials.

BUCHAREST (Reuters) - Romanian President Traian Basescu survived a referendum on his impeachment on Sunday after the voter turnout fell short of the required level and derailed an effort by his opponents to oust him from office.
Leftist Prime Minister Victor Ponta's efforts to unseat the conservative Basescu have brought a stern dressing-down from the European Union, which accused him of undermining the rule of law and intimidating judges.
The row over Basescu has delayed policymaking, sent the leu currency plunging to record lows, and pushed up borrowing costs. It also raised concern about the future of Romania's 5 billion euro ($6.2 billion) International Monetary Fund-led aid deal.
The election bureau said the voter turnout was 46 percent, below the 50 percent threshold Ponta's leftist Social Liberal Union (USL) needed to make the referendum valid.
Exit polls showed more than 80 percent of those who went to the ballot box had voted to remove the president.
"The flame of democracy has remained alight. Romanians have rejected the coup d'etat," Basescu said.
Ponta, whose government took office in May, suspended Basescu and held the referendum to seek popular backing for the impeachment for overstepping his powers. The president is unpopular for backing austerity and for perceptions of cronyism.
The electoral bureau's figures have a margin of error of three percentage points and do not include Romanians voting abroad, but it is now almost certain that final figures - probably on Monday - will show turnout was below 50 percent.
Opinion polls had shown some 65 percent of Romanians wanted to remove the former sea captain from office, but the opposition had called for a boycott of the vote and many people were on holiday.
The president's most important power is nominating the prime minister, which could be crucial after a November election that may leave a split parliament. The president also appoints the chief prosecutor and some judges, including to the Constitutional Court.
That court, which previously said the threshold had to be observed, will make the decision on the vote's validity this week.
"The Romanian government will respect all decisions of the Constitutional Court and will act as a factor of stability in the next period, regardless of whether the referendum is validated or not," Ponta said.
EU PRESSURE
Romania has made progress since the 1989 overthrow of communist dictator Nicolae Ceausescu and joined the EU in 2007, but the economy slipped back into recession in the first quarter of this year and pockets of severe poverty remain.
Ponta felt the full weight of EU wrath after his government took on the Constitutional Court, threatening to replace judges and reduce its powers, and ignoring one of its decisions. Brussels said it was concerned about the government's respect for the rule of law, democratic procedures and the judiciary.
The government had tried to make it easier to impeach Basescu by removing the minimum turnout rule, but was forced to back down following harsh EU criticism and a Constitutional Court ruling that a 50 percent turnout was obligatory.
Basescu initially urged Romanians to vote against what he called a coup d'etat, but this week he changed his mind and he and his allies, the opposition Democrat Liberal Party (PDL), asked supporters to boycott the referendum, citing concern about the possibility of electoral fraud.
"We have an entire political class that puts their own interest before the country's," said pensioner Monica Munteanu. "I am not voting."
Brussels has a wide range of levers with which to put pressure on Romania, whose justice system is under EU monitoring. Romania gets European cash to help it catch up with other members and the bloc contributes to its IMF-led aid deal.
Ponta promised to respect the rule of law and the independence of the judiciary, but Brussels replied that it had yet to see proof of this, for example by the replacement of a USL loyalist with a neutral figure as public ombudsman.
The USL says Basescu - whose term expires in 2014, when he cannot run again - had undue influence over the judiciary after the Constitutional Court ruled against some government laws.
The president can block legislation but only once before parliament can overrule him with a second vote.
"I voted to take him down because he cut my pension and he doesn't deserve to be in power," said Sandu Neacsu, a 66-year-old pensioner from Pantelimon near the capital Bucharest.

JERUSALEM (AP) ? Standing on Israeli soil, U.S. presidential candidate Mitt Romney on Sunday declared Jerusalem to be the capital of the Jewish state and said the United States has "a solemn duty and a moral imperative" to block Iran from achieving nuclear weapons capability.
"Make no mistake, the ayatollahs in Iran are testing our moral defenses. They want to know who will object and who will look the other way," he said. "We will not look away nor will our country ever look away from our passion and commitment to Israel."
The presidential election hovered over the speech. The Old City formed a made-for-television backdrop behind Romney, while some of his campaign donors listened in the audience.
Romney's declaration that Jerusalem is Israel's capital was matter-of-fact and in keeping with claims made by Israeli governments for decades, even though the United States, like other nations, maintains its embassy in Tel Aviv.
He did not say if he would order the embassy moved if he wins the White House, but strongly suggested so in a CNN interview.
"My understanding is the policy of our nation has been a desire to move our embassy ultimately to the capital (Jerusalem)," he said, adding, "I would only want to do so and to select the timing in accordance with the government of Israel."
His remarks on the subject during his speech drew a standing ovation from his audience, which included Sheldon Adelson, the American businessman who has said he will donate millions to help elect Romney to the White House.
Romney's embrace of Israel was on display earlier in the day when he met with Prime Minister Benyamin Netanyahu and other leaders. He also visited the Western Wall, Judaism's holiest site, where he was mobbed by worshippers. In addition, Romney met with Palestinian Prime Minister Salam Fayyad.
In his remarks, Romney steered clear of overt criticism of President Barack Obama, even though he said the threat of a nuclear-armed Iran "has only become worse" in the past five years.
In an unspoken rebuttal to Obama and other critics, Romney said, "It is sometimes said that those who are the most committed to stopping the Iranian regime from security nuclear weapons are reckless and provocative and inviting war.
"The opposite is true. We are the true peacemakers," he said.
The former Massachusetts governor also stepped back from a comment a senior aide made a short while before the speech.
"We recognize Israel's right to defend itself," he told the audience. Earlier, the aide, Dan Senor, previewed the speech for reporters, saying that "if Israel has to take action on its own, in order to stop Iran from developing the capability, the governor would respect that decision."
Israel is the second of three stops on an international trip for Romney in the weeks before he claims the Republican nomination at his party's national convention in Tampa, Fla.
He flew to the Middle East from Britain, where he caused a stir by questioning whether officials there were fully prepared for the Olympic Games. A stop in Poland will complete his trip.
Four years ago, then-U.S. Sen. Barack Obama also visited Israel as a candidate, part of a five-nation trip meant to establish his own foreign policy credentials.
In his speech, Romney said Syrian President Bashar Assad "desperately clings to power" in Damascus in the face of an attempted overthrow, but he did not call for his removal.
He noted that Egypt is now headed by an "Islamist president, chosen in a Democratic election. ... The international community must use its considerable influence to insure that the new government honors the peace agreement with Israel that was signed by the government of Anwar Sadat" more than three decades ago, he said.
A goal of Romney's overseas trip is to demonstrate his confidence on the world stage, but his stop in Israel also was designed to appeal to evangelical voters at home and to cut into Obama's support among Jewish voters and donors. A Gallup survey of Jewish voters released Friday showed Obama with a 68-25 edge over Romney.
Romney and other Republicans have said Obama is insufficiently supportive of Israel, noting statements the president has made about settlements and his handling of evident Iranian attempt to develop nuclear weapons.
Tehran is closer to developing nuclear weapons capability than before, Romney said. "Preventing that outcome must be our highest national security priority."
In a March speech before a pro-Israel lobby in Washington, Obama warned of "loose talk of war" that serves only to drive up oil prices. "Now is not the time to bluster," he said then. "Now is the time to let our increased pressure sink in and sustain the broad international coalition we have built."
It was unlikely that the day's events would settle the issue.
Obama's former press secretary, Robert Gibbs, told ABC's "This Week" that the administration has delayed Iran's nuclear program. The president has imposed U.S. penalties against Iran and worked to tougher strictures applied by other nations. There have been numerous published reports of a coordinated U.S.-Israeli cyberattack that caused damage to Iranian equipment vital to creating weapons-grade nuclear material.
Even so, Israeli Prime Minister Benjamin Netanyahu said before the speech that "all the sanctions and diplomacy so far have not set back the Iranian program by one iota."

Bernard also presented an assessment of global HIV-criminalization hot-spots?a final ranking of 15 nations that had the highest number of HIV-related arrests and prosecutions per 1,000 people living with HIV through July 1, 2012. The United States was at the very top of the list of 15 countries, with the states of South Dakota, Idaho, Mississippi, Louisiana, Tennessee, Illinois, Missouri, Indiana, Ohio and Oklahoma responsible for the greatest number of HIV-criminalization prosecutions and arrests. ?

However, Bernard explained, there is growing evidence that HIV-related prosecutions increase HIV stigmatization and discourage HIV testing and treatment.

Nearly 40 percent of survey participants reported that they?d worried a few times or frequently about being falsely accused of not disclosing their HIV-positive status. ?For HIV people, when things go terribly wrong in a relationship, we can be vulnerable to not only the pain of ending a relationship but also punishment through the legal system,? said Sprague, who was speaking as a researcher and a woman living with HIV. Among transgender people living with HIV, this number was 60 percent. Respondents in the Midwest and the South were more likely to express fear about false accusations.
Respondents also indicated that they did not trust the criminal justice system and 49 percent said that their state would not, or probably not, give them a fair hearing if someone were to file a charge against them for non-disclosure. ?People felt that as soon as they are seen as a person living with HIV, they will not be trusted and will not be seen as a reliable witness,? Sprague said. Another 29 percent of respondents said that they weren?t sure if they would get a fair hearing.
In fact, among the study participants uncertainty about the laws surrounding HIV disclosure was extremely common. More than 60 percent of respondents were not certain whether or not their state required people with HIV to disclose their status to a partner before having sex. Nearly half or respondents also expressed a lack of clarity about what actions or accusations could subject them to prosecutions. ?People living with HIV in the U.S. find themselves in an incredibly vulnerable legal situation in which they can?t predict what kinds of behaviors might be criminalized,? Sprague added.
However, fear of prosecution did not appear to translate into increased disclosure. In fact, fear of criminal prosecution for non-disclosure was low on the list of replies when respondents were asked what factors motivated their HIV disclosure in relationships or potential relationships. Instead, respondents offered these reasons for disclosure: Because it?s the right thing to do; to have honest relationships; to not cause harm to other human beings; and to protect their partners.
These answers, Sprague explained, indicate an ethical- and not a legal-based thought process when considering disclosure.
While Sprague is clear that HIV criminalization appears to be detrimental to public health by discouraging testing, treatment and access to care, she insists that it may play a greater social role. ?Criminalization serves to scapegoat individual people for larger social failures, including the failure to provide social and psychological support for people living with HIV; the failure to deal with social stigma and discrimination; the failure to fund empowerment projects; and the failure to deal with gender inequality? she concluded.
The Personal and Societal Costs of HIV Criminalization
To further explain the personal and psychological results of being a defendant in an HIV-criminalization prosecution, Nick Rhodes discussed his conviction under the Criminal Transmission of HIV Law in Iowa. Rhodes was initially sentenced to 25 years in prison for not disclosing his HIV status during sex, even though he used a condom and HIV was not transmitted. He spent a year in prison, but after pressure from HIV advocates he was given parole and had to register as a lifelong sex offender.

In addition, this early study -- performed in animal models -- shows that tumors fueled by these COPD-induced signals may be especially susceptible to prevention or perhaps even treatment with drugs that turn off these same signals, namely VEGFR-2 and EGFR inhibitors.
"At least in animal models, this study shows an important pathway activated in lung tumors arising in poorly oxygenated regions of the lung that isn't activated to nearly the same degree in other lung cancers," says York Miller, MD, investigator at the CU Cancer Center and professor in the Department of Pulmonary Sciences and Critical Care Medicine at the University of Colorado School of Medicine and Denver Veterans Affairs Medical Center, the paper's senior author.
"There are probably other mechanisms driving lung cancer in COPD as well -- for example, inflammation is also very likely playing in -- but this paper shows that the hypoxic sensing pathway is specifically activated in these COPD lung cancer models and that this sensing pathway is to a large degree driving tumor growth," Miller says.
Specifically, his study used animal models designed to develop cancer, which the group placed in high altitude chambers set to mimic the chronic oxygen depletion of found in parts of the lung affected by COPD. Mice in the hypoxic condition developed larger tumors than mice given normal oxygen, but, according to Miller, what was especially striking is the reason for this tumor growth.
"We saw that tumor growth in the hypoxic environment -- which mimics that of COPD conditions including chronic bronchitis and emphysema -- is due to signaling by HIF-2a. This HIF-2a in turn activates cancer growth promoting mechanisms including VEGF and the EGFR ligand, TGFa, which are growth factors involved in stimulating cell proliferation and the development of new blood vessels," Miller says.
Likewise, just as tumors that arise in hypoxic conditions do so through turning on pathways that lead to the over-production of VEGF and TGFa, so too are these tumors especially susceptible to cancer therapies that block these growth factors. Sure enough, animal models given the drug vandetanib -- a combined VEGFR/EGFR inhibitor -- failed to develop cancer under hypoxic conditions.
"Chemoprevention hasn't been done successfully for lung cancer," Miller says, "but this approach of VEGF/EGFR inhibitors for patients with COPD and extreme lung cancer risk may be something that should be explored further."
Miller imagines the next step is a review of patient records to discover if COPD lung cancer patients who happened to be treated with VEGF/EGFR inhibitors, in fact, had better tumor response than patients with normal lung function and similar tumors.
"Right now it's not a treatment," Miller says, "but it's an exciting line of inquiry."

But the accompanying technical report, running some 52 pages, didn?t come close to making the case that the risk would be worth it. For the most part, it confirmed what critics have been saying all along.
The Enbridge project would mean more tankers and bigger tankers carrying heavy and dirty oil over an environmentally sensitive landscape and exceptionally risky waters. Granted tanker safety has improved and the incidence of serious spills has declined.
Still the report concedes the bloody obvious: ?It only takes one major incident to cripple an ecosystem and incur enormous costs on the responsible party, individuals, local communities and other sectors of the economy and government.?

The implication being that neither Enbridge nor the federal government has treated first nations with sufficient respect.
Not surprisingly, as Polak noted during her comments to reporters, virtually every native representative who has spoken to date at the National Energy Board review of the project has been opposed. ?I can?t think of one first nation that supports it.?
She stopped short of saying that individual bands have a veto over the project. In practical terms it amounts to the same thing: With this degree of opposition from first nations, the line could be tied up in blockades and court battles for a generation.

Not much, when it comes right down to it. The report says that in terms of economic and fiscal benefits, B.C. stands to reap about eight per cent of the projected increase in government revenues, and double that share in gross domestic product.
Those benefits would be spread over 30 years. On an annualized basis, figure maybe $200 million in additional revenues, $1.5 billion on the GDP.
All in exchange for assuming a significant share of the risk of a spill on land (58 per cent, based on the portion of the line that would run through B.C. territory) and fully 100 per cent of the risk of a spill in coastal waters.
For all that, the Liberals maintained that the province could be persuaded to ?consider support? were there more resources to fight spills, a better deal for natives and more benefits to the provincial treasury.
But I doubt the Albertans, or the company, or the federal government will be inclined to put out to the degree necessary to satisfy the concerns raised in the report.
Realistically, the technical analysis amounts to a persuasive argument for opposing the Northern Gateway project. Reading it, I expect many British Columbians will say to their government, ?What took you so long??

Capital One Bank, in partnership with the Urban League of Greater New Orleans, recently announced the second annual ?Grow Gentilly? small business growth plan competition to help small businesses in the Gentilly area of New Orleans grow and create jobs.
The winner of the Grow Gentilly competition will receive a cash award of $10,000. The contestants who finish second and third will receive $7,000 and $3,000, respectively. The top three also will receive technical and professional assistance to enhance their businesses.
To be eligible for the Grow Gentilly competition, businesses and their owners must meet the following key criteria:
? Business revenues in the most recent fiscal year must have been $1 million or less.
? The applicant must have owned the business since before August 31.
? The business must reside in zip code 70122.
? The applicant/owner must demonstrate the potential or desire to grow and create jobs in the Gentilly community.
The competition is divided into two rounds. In the first, applicants will complete and submit an online form to provide a comprehensive overview of their business, including its mission and vision, products and services offered, management plan, marketing strategy and other key information. Each of the applicants then will be assigned a Capital One business banker who will provide insight and help the applicant fine-tune the application. The applicants will then be expected to re-submit their applications after the one-on-on business banker consultation by Aug. 31. Six finalists then will be announced on September 15 and will compete in the second and final round on October 15, during which each contender will make an eight- to 10-minute business presentation in front of a panel of community and business leaders. The cash awards will go to the owners of the three businesses whose plans are judged by the panel to be the most creative, innovative and likely to succeed and create jobs in the Gentilly area.
The deadline to submit the first-round online application is 5 p.m., July 31, 2012. Applications will be accepted at www.cyber?grants.com/capitalone/growgentilly. The site provides detailed information about the competition, eligibility guidelines and a pre-screening checklist. The re?vised applications must be submitted no later than August 31.
